A psychodynamic note for therapists working with relational patterns, affect, defences, internal conflict, transference, and emerging formulation. This template is designed to help capture the underlying emotional and relational process of the session, including links between present difficulties and earlier or repeated patterns where these are clearly evidenced. It aims to produce a therapist-facing note that supports psychodynamic reflection, formulation, and continuity between sessions while using tentative, grounded, and non-pathologising language.

Session Summary

The session focused on the client’s wish for closeness with a friend and the fear that asking directly for support would feel humiliating. The central conflict explored was between wanting care and anticipating judgement for needing it. The therapist linked the client’s self-criticism after asking for support with a repeated expectation of dismissal, and the client partly endorsed this while saying they were not sure where the pattern began.

Current Presentation and Central Affect

The client entered the session describing anxiety before asking a friend for support and shame afterwards about having made too much of the issue.

Session Content and Central Themes

The central psychodynamic theme was the conflict between wanting care and fearing that visible need would be excessive or burdensome. A recent request for support from a friend was used as a brief cue to explore dependency, shame, and uncertainty about whether needing others is acceptable.

Relational Patterns and Internal Conflicts

The client identified a repeated pattern of wanting support, asking in a very cautious way, then criticising themselves afterwards. The therapist wondered whether the client expects care to be withdrawn if their need is too visible. The client said this fitted friendships more strongly than the relationship with their sibling.

Defences and Affect Regulation

The session explored self-criticism as a possible protective response after asking for care. The therapist framed this tentatively as a way of pre-empting imagined judgement from others. The client agreed that criticising themselves can make them feel briefly more in control, but said it also leaves them isolated and reluctant to ask again.

Therapeutic Relationship / Transference and Countertransference

The client said they had considered cancelling the session because they worried the therapist would think the friendship issue was minor. The therapist explored this expectation as it appeared in the therapeutic relationship. The client said the worry resembled the fear that other people will minimise their needs.

Therapist Interventions and Client Response

The therapist clarified the sequence from asking for support to later self-attack, reflected the conflict between need and shame, and tentatively linked the client’s expectation of minimisation with their hesitation to bring the issue to therapy. The client said the link made sense but also wanted to avoid assuming that every friendship concern came from the same pattern.

Emerging Formulation

A tentative formulation is that visible need may evoke shame and anticipation of dismissal, leading the client to make very cautious bids for support and then criticise themselves afterwards. The client reported this pattern in the current friendship example and partly connected it with earlier experiences of being told they were overly sensitive, while the origins and scope remain uncertain.

Progress, Repetition and Ongoing Focus

Progress was shown in the client’s ability to ask the friend directly for time to talk and to examine the shame that followed rather than treating it as proof they had done something wrong. Repetition remains in the post-contact self-criticism and the expectation that needing support will be judged.

Plan / Ongoing Therapeutic Focus

• Theme or affect to revisit: Shame after asking for support and the wish to be cared for without feeling exposed.

• Relational or therapeutic-process issue to monitor: The client’s worry that the therapist may minimise apparently minor relational concerns.

• Client reflection or action: Notice whether self-criticism appears after future bids for support and what it seems to protect against.
