Session Summary

The highest-priority target was a self-harm urge following the cancellation of plans with the client’s sibling; the client reported no self-harm and used distress-tolerance skills and social support while the urge reduced. The therapist and client completed a behaviour chain, identified sleep loss, not eating and rapid withdrawal as relevant links, and developed alternatives at earlier points in the sequence. The client agreed to continue diary-card monitoring and use STOP followed by selected TIPP skills when urges begin to rise.

Current Presentation, Interval Events and Diary Card

The client reported current distress of 4/10 and reviewed a diary card completed on six of the previous seven days. It recorded one self-harm urge reaching 7/10, no self-harm behaviour on the completed days, and use of skills on four days; no diary-card data were available for the remaining day. The client reported no suicidal behaviour since the previous session and described reduced sleep and an irregular evening meal before the high-urge episode.

DBT Target Hierarchy and Session Focus

The therapist explicitly prioritised the interval self-harm urge as a life-threatening target because it was part of the client’s established treatment hierarchy, while preserving that no self-harm behaviour occurred in this episode. Withdrawal from supportive contact was identified as a related quality-of-life target. Skills acquisition focused on recognising the escalation earlier and using distress-tolerance skills before moving into isolation.

Behaviour Chain Analysis

The client identified four hours of sleep and not having eaten since lunchtime as vulnerability factors. The prompting event was a message from their sibling cancelling a planned visit. Supported links included interpreting the cancellation as evidence that they did not matter, sadness rated 6/10, anger rated 8/10, an urge to cut off contact, going alone to the bedroom and a self-harm urge that rose to 7/10. The client then noticed their diary-card reminder, used STOP, applied cold water and paced breathing, and contacted a friend. They reported that the urge reduced to 3/10 over approximately twenty minutes and that no self-harm occurred. The therapist and client treated these as the client’s reported sequence rather than a complete causal explanation.

Solution Analysis, Repair and Alternative Links

The therapist and client identified possible change points before isolation: noticing sleep and food-related vulnerability, pausing before interpreting the cancellation, keeping the written crisis-skill sequence accessible, and contacting support before going to the bedroom. They rehearsed a brief plan to use STOP when the urge reached 5/10 and then choose between paced breathing, cold water and contacting the identified friend. A later message to the sibling was explored as a possible interpersonal repair, but it was not assigned or treated as necessary for managing the immediate risk.

DBT Skills Acquisition, Practice and Generalisation

STOP and selected TIPP components were reviewed in relation to the chain. The client reported using both during the interval episode and associated them with enough reduction in urge intensity to contact a friend, while remaining unsure which element helped most. In session, the therapist coached paced breathing and the client reported that the slower exhale was easier to follow than counting equal breaths. This was recorded as an in-session preference rather than evidence of independent mastery.

DBT Strategies, Commitment and Contingencies

The therapist validated the intensity of the hurt and anger while maintaining a change focus on intervening before withdrawal narrowed the client’s options. A brief pros-and-cons commitment strategy was used to compare following the crisis-skill sequence with acting on the urge. The client identified immediate relief as the anticipated short-term consequence of self-harm and increased concealment and shame as likely later consequences, then recommitted to trying the agreed sequence before acting on future urges.

Progress, Obstacles and Treatment Targets

The client reported using skills and support instead of self-harm during one high-urge episode, providing a specific instance of progress within the current treatment hierarchy. The urge nevertheless reached 7/10, and the client noticed the written reminder only after isolating, leaving earlier recognition as an ongoing target. The incomplete diary-card day was recorded as missing information rather than evidence that urges or behaviours were absent.

Therapeutic Relationship / Treatment Review

The client said the therapist’s initial description of the episode as a success felt as though the severity of the urge had been minimised. The therapist acknowledged this, validated the effort and distress separately, and revised the language to recognise both the absence of self-harm and the continuing risk represented by the intense urge. The client said this distinction fitted their experience more accurately.

Between-Session Practice and Next Steps

• Target hierarchy / next-session focus: Continue work on the life-threatening target by reviewing earlier links in high-urge chains and the timing of skill use.

• Client DBT skills practice or behavioural action: The client agreed to use STOP and then the selected crisis-skill sequence when self-harm urges reached 5/10, recording which step was used and the urge rating afterwards.

• Diary-card or target monitoring: Continue daily recording of self-harm urges, target behaviour, suicidal ideation and skills used, leaving any uncompleted day blank rather than estimating retrospectively.

• Crisis, coaching or support plan reviewed or agreed: The client agreed to contact the identified friend or the crisis service in their existing safety plan and to seek emergency help if unable to maintain safety.

• Booking, timing or frequency arrangements: The next weekly session was retained for the agreed time.

Risk, Crisis and Safety Management

The client reported one interval self-harm urge reaching 7/10 and denied acting on it. During direct current assessment, they denied suicidal ideation, intent or plan, reported a current self-harm urge of 1/10 and denied thoughts of harming others. The therapist reviewed the existing safety plan, including the identified friend, crisis-service contact and emergency escalation if the client could not maintain safety. No risk level was assigned in the session.
