Couples Therapy Note

Interaction patterns, partner perspectives, interventions, relationship goals, and shared practice.

About this note

A couples-therapy note for therapists working with relationship distress, communication patterns, conflict cycles, emotional connection, repair, and shared goals. This template captures the couple's central therapeutic work, individual perspectives, interactional formulation, couples interventions, responses, progress, relational safety, and agreed between-session practice.

Example note

Session Summary

The session focused on a recurring conflict about evening household responsibilities. One partner described raising concerns urgently after feeling unsupported, while the other described hearing criticism and becoming defensive or quiet. The therapist helped the couple map this escalation-withdrawal cycle as their shared problem, slowed one exchange, and supported each partner to state an underlying need. Both partners agreed to practise a brief structured check-in between sessions.

Current Relationship Focus and Developments

The couple reported that practical conversations had continued to escalate during a demanding fortnight. They also described one occasion when they paused the argument and returned to it later without shouting, which they wanted to understand and repeat.

Session Content and Relational Themes

The central work involved the meanings each partner attached to the other’s response during discussions about shared responsibilities. Partner 1 described urgency as an attempt to secure teamwork and reassurance that the load was noticed. Partner 2 described rapid questioning as a sign that their efforts had already been judged inadequate, leading them to explain, defend, and then disengage. The couple explored how these protective responses obscured their shared wish to feel like a team.

Interactional Patterns and Working Formulation

The therapist proposed a tentative cycle in which accumulated frustration leads Partner 1 to raise several concerns at once; Partner 2 experiences criticism and responds with explanation or silence; Partner 1 then experiences the withdrawal as further evidence of being alone with the problem and escalates. Both partners recognised the sequence, while Partner 2 added that withdrawal was sometimes an effort to prevent saying something hurtful rather than a lack of care. The formulation remained focused on the interaction rather than locating the problem in either partner.

Partner Perspectives and Areas of Alignment / Difference

Partner 1 wanted shared responsibility to be acknowledged before solutions were discussed. Partner 2 wanted time to understand a request without their pause being interpreted as indifference. They differed about how often the pattern occurred but agreed that defensiveness and urgency left neither feeling heard. Both identified teamwork and calmer repair as relationship goals.

Progress, Obstacles and Relationship Goals

The couple identified progress in interrupting one recent escalation and returning to the topic after a pause. They attributed this partly to Partner 2 stating when they would resume the conversation and Partner 1 not continuing the discussion during the break. A remaining obstacle was the speed with which each interpreted the other’s protective response as proof of not caring.

Couples Interventions and Responses

The therapist used cycle mapping, reframed the cycle as the shared problem, and facilitated a slowed exchange using observation, feeling, need, and request. Partner 1 reported feeling less alone when their need for recognition was reflected. Partner 2 said that hearing the need without several examples at once made it easier to remain engaged. Both found the structure somewhat formal but agreed it was useful enough to practise.

Between-Session Practice and Next Steps

• Treatment direction / next-session focus: Review the couple’s use of pauses and repair, and continue work on expressing needs before urgency and defensiveness take over.

• Joint between-session practice: The couple agreed to hold two ten-minute household check-ins, with each person reflecting what they heard before responding and either partner able to request a timed pause.

• Partner-specific practice: Partner 1 agreed to bring one priority at a time; Partner 2 agreed to state when they needed a pause and when they would return to the conversation.

• Booking, timing or frequency arrangements: The couple kept the next appointment for the usual time.

Risk, Safeguarding and Relational Safety

The therapist asked each partner directly about current fear of the other, threats, physical aggression, and feeling unable to disagree safely. Both denied these concerns. No child-safeguarding concern was disclosed or identified in the discussion.

Client: Hi. Um… I’m okay, I think. A little scattered. I almost forgot we had this today, and then I was rushing to get here on time. Therapist: So you’re coming in feeling a bit rushed and scattered. Client: Yeah. That’s probably the word. Scattered. Therapist: Let’s take a moment, then. No need to jump right in. Maybe just notice that you’re here now. You made it. Client: Yeah. Therapist: As you sit here, what are you aware of? In your body, your thoughts, your mood? Client: My shoulders are tense. I feel like I’ve been holding them up all day. And my mind is kind of jumping around. Therapist: Mm-hmm. A lot of mental movement, and your body is carrying some tension too. Client: Exactly.
